The part that gets missed

It was never only about libido

Testosterone gets filed under sex drive. That's the one approved indication, that's what the trials measured, and that's what a doctor hears when you raise it. So if your complaint isn't desire, you never get offered it.

The symptom I care about is the flatness. Not sadness, and not tiredness. The thing where you do what you used to love and notice, somewhere in the middle, that nothing is reaching back. Clinically it's called anhedonia.

I had it for thirty years while being treated for depression the whole time. That isn't a failure of my medication. Antidepressants work on serotonin, and this kind of flatness runs on dopamine, which is a system testosterone acts on. Treating one does nothing for the other.

I won't tell you testosterone is a proven treatment for it, because the trial that would prove it hasn't been run, and you should distrust anyone who tells you otherwise. What I'll tell you is that the question is real, it's unanswered, and meanwhile women describing exactly this are being sorted into "depression" without anyone checking.

Why you weren't told

The research didn't fail. It asked the wrong questions.

This isn't a conspiracy story. It's a methods story, and the methods are genuinely bad in ways that are easy to show.

Assays that can't see you

Eleven professional bodies concluded in 2019 that the standard tests are "highly unreliable in the female range." That's their highest grade of evidence, not a fringe view.

Doses drawn from men

The clearest trial showing testosterone builds muscle in women got its results at levels above the female range. That isn't what a guideline-following doctor would prescribe.

The wrong outcome measured

The one trial usually cited to close the mood question used a general depression scale and no measure of flatness at all. Its own authors flagged that a 49% placebo response may have hidden any effect.

No product, so no budget

Nobody sells an FDA-approved female-dosed testosterone product in the US. With no product there's no marketing budget, and with no marketing budget nobody funds the answer.

I'm not a doctor. I'm the person who runs the tests.

I'm a medical technologist. I've drawn blood and run labs for years, which is exactly why the measurement problems on this site bother me so much. I read primary research, and when I cite a study I'll tell you who was in it, what was measured, and how many people it took.

I'm also in this personally. I carry two copies of APOE4, I've been on hormone therapy through menopause, and I'm on testosterone. My own estradiol was too low for a long time and nobody ever told me. I only caught it because I know what the number should look like. Most women don't have that background, which is the entire reason this site exists.
